The information should include: your first & last name, mailing address, and RN or Dr’s License number, email address, and phone number (optional) so we can verify that you are a registered DR or a Nurse and we will mail out your KN95 mask today from our distribution center in New Jersey. Program starts April 1'st untill supply lasts.
